Thank you very much for performing cross-border traveling with the DIH-HERO travel voucher. For the reporting of the cross-border travel activities that have been undertaken with the DIH-HERO travel voucher the following information will be required:

1. General contact information and bank details
2. Description of the cross-border activities undertaken (including elaboration on necessity, the connection to the DIH-HERO network the travel sequence, the actual outcome and impact on the organisation)
3. Upload of receipt(s) and proof of payment(s) for each described travel activity
4. Informed Consent for data processing and storage (via the link given in the form)
